年に退院支援専従看護師の配置、退院支援委員会を立ち上げ、現場の看護師が、退院支援における看護師の役割について理解し、実践に繋がるように取り組んだ。また2010年から取り組み始めた新看護体制（総リーダーを配置した固定チーム継続受け持ち制） の中で総リーダーを退院支援のキーパーソンとして、役割が果たせるよう活動を開始した。今回、5年間の取り組みの評価として、看護師の意識変化と現在の課題を明らかにした。その結果、退院支援に対する看護師の意識は高まっており、全体でみると、師長、副師長、ラダーⅣ、ラダーⅢ、ラダーⅡ、ラダーⅠ、ラダー0
