{"created":"2021-03-01T06:11:57.780238+00:00","id":8107,"links":{},"metadata":{"_buckets":{"deposit":"a1d8275b-cce7-4818-ac6f-a47254197d3a"},"_deposit":{"id":"8107","owners":[],"pid":{"revision_id":0,"type":"depid","value":"8107"},"status":"published"},"_oai":{"id":"oai:soar-ir.repo.nii.ac.jp:00008107","sets":["882:884:885:886"]},"author_link":["22669","22670","22671","22672","22673"],"item_10_biblio_info_6":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2014-03","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"13","bibliographicPageStart":"9","bibliographicVolumeNumber":"42","bibliographic_titles":[{"bibliographic_title":"信州大学医学部附属病院看護研究集録"}]}]},"item_10_description_20":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"脳外科術後で高次脳機能障害や身体障害のある患者が,自宅退院するにあたり,当院では外泊を行い退院の準備を整えて,退院するケースが多かった。初回の外泊中は自宅での生活動作・生活環境の状況を把握するのに重要な情報源となる。しかし,外泊中の状況を経験や知識により看護師が,的確に情報を得られる事が出来ていなかった。看護師の経験にとらわれず,誰もが必要な情報が得られるような用紙が必要と考えた。自宅退院に向けて外泊情報収集シートを作成した。そこで日常生活能力・自己管理能力・安全管理能力の項目に分け,実施状況をA: 「問題なく1人で出来た」 B: 「促し指示を出す必要があった」 C: 「手助けが必要」と3段階に分けた自立度が評価できるように,外泊情報収集シートを作成した。外泊情報収集シートを使用し,外泊を経てから面接をする事で,患者が入院生活では出来ている事が自宅では出来たのか,確認できた。また,面接を通して外泊中の家での患者・家族の不安をきくことができた。看護師同士が情報を共有して,統一した看護を提供することができた。外泊情報収集シートはADL (日常生活動作).
