"終末期,モルヒネの大量投与を行ったが,癌性疼痛のコントロールが難しかった患者の看護を経験した。モルヒネの投与量と疼痛,及び,副作用症状の主なものとして嘔気嘔吐,排便状況,呼吸抑制について対処薬剤の使用方法を含め看護の経過を見直した。症例患者は,経口薬4500mg,注射薬120mgの併用で疼痛の改善は見られたが,安定した除痛は難しかった。腫瘍の神経系の圧迫や脳への転移,脳圧の上昇などがある場合,モルヒネのみでの疼痛コントロールは困難だが,対症薬剤や鎮痛補助薬を併用,ケアすることでモルヒネの効果も上がる。モルヒネ投与後の十分な観察,副作用症状に対し予防策や対処方法をとるなど適切な投与方法と管理を行えば,ターミナル期でもモルヒネの大量投与は行え,患者の苦痛の緩和につながる。",
