"研究の目的は,胃ろう造設に対する患者とその家族の思いを明らかにすることである。A病棟では,疾患による燕下障害で経口摂取が困難となり,胃ろう造設を選択する患者が多い。また,胃ろう造設をすでに決定し入院してくる患者がほとんどであり,病棟看護師が胃ろう造設を選択した患者とその家族の思いを知る機会は少ない。胃ろう造設を選択した患者とその家族3組を対象に,入院時に半構成的面接を行い,データを収集し,得られた内容を分析した。その結果,胃ろう造設や疾患への思いに関して124のコードが抽出され,それらを40のサブカテゴリーに分け,最終的に【不安】 【葛藤】 【期待】の3つのカテゴリーで構成されていると考えられた。胃ろう造設を選択した患者とその家族は,胃ろう造設術に対する不安の他にも,疾患の進行や今後の生活,介護に対する不安を抱えていた。また,患者自身の思いと家族を気遣う気持ち,家族間で意見が合わないこと等,胃ろう造設を選択するまでに大きな葛藤もみられた。その一方で,胃ろう造設後胃ろうを使用することで,体力が向上することを期待しており,疾患の治癒に対する期待も抱いていた。インタビューを通して胃ろう造設に対する思いを傾聴する事で,胃ろう造設の決定には患者本人だけでなく,家族の思いも大きく関わっている事がわかった。それぞれの家族には胃ろう造設を決定するにあたり,疾患・介護・今後の生活についての価値観があり,その思いを明かにすることで,胃ろう造設時の不安の軽減やその後の胃ろう管理の指導を行う時に,スムーズな関係を築いていけると考えられた。",
