[{"subitem_description": "要旨：飯田市では平成22 年度に健康福祉に係る独自計画「地域健康ケア計画」を策定した。本計画の重点プロジェクトの一つ「健康づくり家庭訪問」は、訪問等を通して健康づくりのアドバイスを行うとともに、住民の生活や健康の実態を把握してニーズに合った保健事業につなげることとしている。平成22、23 年度はモデル地区で全年代を対象に実施し、平成24 年度からは全市展開として退職等で社会的、身体的に変化の多いと思われる62 歳を対象に実施した。62 歳の訪問による結果から、62 歳より前に重篤な生活習慣病を発症している人が多い現状が見えてきた。今後は市民や地域と協働して働き盛り世代への生活習慣病予防、特定健診受診者の生活習慣病重症化予防の取り組みを強化していきたい。", "subitem_description_type":
