"目的:定点外医療機関からの情報を契機に、麻しんの新たな発生防止と地域内の感染拡大防止対策を講じた。この取り組みの経緯と内容を紹介し、取り組みの過程で明らかとなった保健所の役割を整理する。方法:(1)医療機関や学校等からの任意報告による発生状況の把握、(2)把握した患者および接触者の健康調査とまん延防止対策、(3)感染症発生動向調査情報等による市町村及び医師会等医療・教育機関への情報提供と注意喚起、相談窓口の設置、を行った。結果:定点報告により発生動向を把握している麻しんは、週報のため地域内の発生状況の把握に時間的な遅れをとり、流行予測の迅速な判断や早期対応に限りがある。そのため、任意報告による発生状況を根拠とした活動を展開した。また、福祉施設や学校等の感染症対策は、新たな発生予防やまん延防止に対する備えが不十分であった。平常時からの情報管理や各施設の「対応マニュアル」整備への働きかけなど、発生予防とまん延防止に重点を置いた事前対応策の強化が必要であると示唆された。結論:保健所の役割は、発生時の対応だけでなく、日頃からの感染症の予防や地域の流行につながる可能性のある事象を早期に把握し、まん延を防止していくことである。この事前対応型の機能を発揮するため、地域に視点を置いた情報管理や人材育成など市町村や各機関が共有する地域版健康危機管理体制の強化・再整備が必要であると考える。",
