"我々は術中の皮膚保護に対し平成10年度よりコムフィール[○!R],デュオアクティブ[○!R]などの皮膚保護剤の導入を行った。皮膚損傷予防に対する研究で,腹臥位,側臥位での皮膚損傷の発生要因を調査した結果,46%に皮膚損傷を認めた。手術時の皮膚損傷は患者の年齢,体重,生理的因子(栄養状態など),皮膚保護剤の使用の有無よりむしろ手術時間に左右される事が明らかになった。そこで今回,長時間手術が予想される脳神経外科の腹臥位手術と整形外科の側臥位手術で,あえて皮膚保護剤の使用をせず除圧材料のみを用いてケアを行った。その結果,長時間手術に対しては体位設定の工夫を行い,術者と共に検討し除圧材料を効果的に用いて圧分散を計れば,皮膚損傷を発生させない事が確認できた。更に,スタッフ誰もが一定に個々の患者の,体型,皮膚の状態を観察しアセスメントできる\"\"スケール\"\"の必要性を感じた。",
